Once an intake session is scheduled, I'll provide you with the forms you'll need to complete in order to begin services. This includes a consent for services form, an intake form, and when applicable, the sliding scale policy. I'll distribute my Professional Disclosure Statement, which is also found below. This is a document that I am required to give to all clients before beginning services.
Please note that the intake form asks for a fair amount of information, so it's best to allow for adequate time and self-care (if needed) when completing it.
