New Clients

It's normal to be a little (or a lot!) anxious for your first counseling session. I try to not add to that anxiety with the paperwork process. The forms needed for your first session are found at the bottom of this page.

Many people find it helpful to complete paperwork before the first session. For confidentiality reasons, do not email them. Completing paperwork ahead of time allows for more time to talk about your needs and hopes for counseling during the first session.

If you aren't sure about a question (or an entire form), just leave it blank and we can complete it together during session. 

If you have access to a printer, print out the appropriate Consent for Services and the appropriate Intake Form found below. If you qualify for a sliding scale fee, print out and complete this form. As well, please read the Professional Disclosure Statement. We will review and discuss it in session.

As a reminder, payment is due at the end of each session. During COVID-19 protocol, it is preferable if you can pay by debit/credit card. I have mobile payment options that make contactless payment quick and easy. If you need to make arrangements to pay by cash or check, please let me know. I am not able to bill insurance at this time. If you need to cancel or reschedule your first appointment, please contact me at least 24 hours in advance. If it is less than 24 hours, please contact me anyway! Any notice is more helpful to me than no contact/no show. You can find my contact information here.

Below you will find a description when we are in typical face-to-face format.

My practice is located within the Breakthrough Family Counseling office. Some clients are surprised to discover the office is a converted house, so I find that helpful information to mention here. There is pull-in street parking on Cornell Street to the side of the office, just around the corner from Berkeley Street. Please park in these available spaces. 

 

When you arrive, there is no need to knock. Enter through the front door and take a seat in our waiting area. I will greet you at your session time!
